María del Rosario Cayetana Fitz-James Stuart y Silva, 18th Duchess of Alba GE (28 March 1926 – 20 November 2014) was one of the most senior aristocrats in Spain, as well as the most titled aristocrat in the world, title now held by the Princess Victoria of Hohenlohe-Langenburg, 20th Duchess of Medinaceli.
Born at Liria Palace in Madrid on 28 March 1926, Cayetana Fitz-James Stuart was the only child of the 17th Duke of Alba (a prominent Spanish politician and diplomat during the 1930s and 1940s) by his wife, María del Rosario de Silva y Gurtubay, 9th Marchioness of San Vicente del Barco. She was the 8x granddaughter of James II. Her godmother was Queen Victoria Eugenie, the wife of King Alfonso XIII of Spain.
As a socialite, the duchess met famous VIPs from Spain and abroad. Jackie Kennedy visited her Seville palace, as did Wallis Simpson, Grace Kelly and Prince Rainier III of Monaco. In 1959 the duchess, together with designer Yves Saint Laurent, hosted a Dior show for charitable purposes in her Liria Palace, Madrid, a palace which movie stars Charlton Heston, Sophia Loren, Audrey Hepburn and Raf Vallone visited. In her youth the duchess posed for Richard Avedon and Cecil Beaton and she appeared on the cover of Time and Harper’s Bazaar.

Fue bautizada el 17 de abril en el Palacio Real, siendo sus padrinos el rey Alfonso XIII y su esposa la reina, Victoria Eugenia. Para su bautizo se trajo la pila bautismal de Santo Domingo de Guzmán, una pila que solo se utilizaba para bautizar a monarcas o sus descendientes. Recibió los nombres de María del Rosario Cayetana Paloma Alfonsa Victoria Eugenia Fernanda Teresa Francisca de Paula Lourdes Antonia Josefa Fausta Rita Castor Dorotea Santa Esperanza Fitz-James Stuart y de Silva Falcó y Gurtubay.[8]​ De sus nombres, ella siempre ha dicho que prefería el de Cayetana, y que el que menos le gustaba es el de Alfonsa, aunque era en honor a su padrino.
Cuando los reyes partieron al exilio, el 14 de abril de 1931, el duque de Alba decidió salir también rumbo a París. Allí se trasladaron al hotel Prince of Galles, cerca de donde vivía Alfonso XIII que también estaba allí exiliado. Cayetana no tuvo muy buenos recuerdos de su vida en París, ni de su colegio. En París, fue operada de apendicitis. Aunque en el momento de su Primera Comunión residían en París, se trasladaron a Madrid para efectuarla en la capilla del Palacio de Liria. Cayetana tuvo pocos recuerdos de ese día, recordó su vestido blanco, su pelo alisado y un tobogán que le regalaron y que estuvo mucho tiempo instalado en los jardines de Liria.
